Health Services Forms

Please see below for Health Services forms. Though most fields can be completed online and saved, many require a physical signature. Please check each form for requirements.

For help with forms, please reach out to your child's school nurse or email FWPSHealth@fwps.org.

School Entrance and Immunization Forms

To attend school, students must have an up-to-date, medically verified immunization record or Certificate of Exemption on file with their child's school. (WAC 246-105-030 and WAC 246-105-040)

💡 Remember, you can view and print official state immunization records through MyIR Mobile.

Medication Authorization Forms

If your child will need to take or be given medication at school, you must fill out the appropriate medication form. In some cases, a Prescription Medication Order Form is required. To learn more about our medication policy, please see the webpage Medications at School.

Condition Specific Forms

If your child has a life-threatening or significant health condition requiring specific treatment or management during school hours, you must complete the relevant order forms so that your child's school nurse can create and implement a Care Plan.

Release of Health Records

Coordinating care can be tricky and time-consuming. If you'd like us to speak to your child's doctor's office to obtain records, medication orders, or to plan care, we will need a HIPAA exchange of information form filled out.
