Prearranged Absence Request

Our district requires WSP background checks for anybody that would like to volunteer, go on a field trip or be working with kids at anytime in the building.  You can find this form at the link below or can pick up a copy in the office.  Your application must be submitted in-person as we are required to verify and photocopy your state identification.
